Qualifications

• Must hold a state CSW or LSW certification.
• Bachelor’s in social work or in a human services field.
• Experience with MDS 3.0, PHQ-9 and BIMs in long‑term care or sub‑acute settings preferred.

Responsibilities

• Performs social services based on facility policies and regulations.
• Complete initial Social Service History within 14 days of admission.
• Maintain and update care plans and MDS components in a timely manner.
• Coordinate discharge planning and family communications.
• Assist with resident satisfaction surveys and group facilitation.
