Rose Mountain Care Center in New Brunswick, NJ seeks a Full Time licensed Social Services Director with experience to lead our social services team in a facility setting. The role requires a CSW/LSW certification and strong knowledge of regulatory requirements and care planning.

Responsibilities include assessments, care planning, documentation, discharge planning, and coordinating with families and the NASW Code of Ethics to maximize resident well-being and independence.

Formación

• Graduate of an accredited school in social work or related field.
• Bachelor’s degree in Social Work or a related human services field.
• State certified by the Board of Social Work Examiners as CSW or LSW.
• Experience with MDS 3.0 / PHQ-9 / BIMs in long-term care or sub-acute settings preferred.

Responsabilidades

• Perform assessments related to psychosocial functioning, cognitive status, and adjustment needs.
• Complete documentation: initial social work history within 14 days of admission, MDS 3.0, CAA’s, IDCP, care plan updates, progress notes, hospital updates within 48 hours, discharge planning, family contact, and resident satisfaction surveys.
• Participate in interdisciplinary meetings; ensure compliance with federal/DOH/OBRA regulations.
• Assist with facility-wide responsibilities: courtesy, confidentiality, teamwork, and flexibility.
