Job summary

In this role you will develop budgets, manage resources, document care interactions, and build partnerships with community organizations to expand access to support.

Qualifications

• Bachelor's degree in Social Work, Healthcare Administration, or a related field (or equivalent experience)
• Excellent communication and interpersonal skills with the ability to build rapport with patients, families, and healthcare teams
• Strong leadership and management skills with the ability to mentor and supervise staff
• Ability to work in a fast-paced environment with multiple priorities and deadlines
• Proficiency in electronic medical records and other healthcare software

Responsibilities

• Develop and implement social services programs and services to meet the needs of patients and families
• Collaborate with healthcare teams to identify and address social determinants of health that impact patient care
• Recruit, train, and supervise a team of social workers and patient navigators
• Develop and manage departmental budgets and resources
• Ensure compliance with regulatory requirements and accreditation standards
• Foster strong relationships with community partners and stakeholders to expand services and resources
• Maintain accurate records and documentation of patient interactions and services provided
