Job summary

Signature HealthCARE is seeking a Social Services Director in the Lexington, KY area to identify and address residents’ social, emotional, and psychological needs throughout their stay and to assist in discharge planning.

The role requires a Bachelor’s or Master’s in Social Work or Psychology and at least 3 years of experience in social services within a healthcare setting, along with a high level of professionalism and confidentiality.

• Bachelor’s or Masters Degree in Social Work or Psychology.
• 3+ Years Experience working in Social Services, directly with individuals, preferably in a healthcare setting.
• High level of professionalism and ability to maintain confidentiality.

Responsibilities

• Identify and provide for each resident’s social, emotional and psychological needs.
• Assist in planning for discharge and care planning.
• Complete documentation to comply with Facility, State, and Federal policies.
• Participate in resident care planning and discharge planning.

Overview

As the Social Services Director you will identify and provide for each resident’s social, emotional and psychological needs, and the continuing development of the resident’s full potential during his/her stay at the facility and to assist in the planning for his/her discharge.

How You Will Make a Difference

• Assist residents and their families with social, emotional, and family issues.
• Provide timely and accurate completion of documentation needed to comply with Facility, State, and Federal policies.
• Participate in resident care planning.
• Participate in the development of Discharge planning and prepare resident for the transition.

  • Bachelor’s or Masters Degree in Social Work or Psychology.
  • 3+ Years Experience working in Social Services, directly with individuals, preferably in a healthcare setting.

  Salary Range

  USD $25.65 - USD $30.50 /Hr.
