Job summary

Signature HealthCARE asks for a Social Services Director to identify and address residents’ social, emotional, and psychological needs throughout their stay at the Spencer County facility. You will help in discharge planning and coordinating transitions, while ensuring all documentation complies with applicable policies.

Qualified candidates have a Bachelor’s or Master’s in Social Work or Psychology, 3+ years in social services within healthcare, and a strong commitment to confidentiality and

• 3+ years experience working in Social Services, directly with individuals, preferably in a healthcare setting.
• High level of professionalism and ability to maintain confidentiality.

Responsibilities

• Identify and provide for residents’ social, emotional and psychological needs during their stay.
• Assist in planning for discharge and transition.
• Provide timely, accurate documentation to comply with facility, state, and federal policies.
• Participate in resident care planning and discharge planning.
• Serve as a resource for residents and families.

Overview

As the Social Services Director you will identify and provide for each resident’s social, emotional and psychological needs, and the continuing development of the resident’s full potential during his/her stay at the facility and to assist in the planning for his/her discharge.

How you Will make a difference

• Assist residents and their families with social, emotional, and family issues.
• Provide timely and accurate completion of documentation needed to comply with Facility, State, and Federal policies.
• Participate in resident care planning.
• Participate in the development of Discharge planning and prepare resident for the transition.
• Serve as a general source of information for residents and their families.

What you Need to make a Difference

• Bachelor’s or Masters Degree in Social Work or Psychology.
• 3+ Years Experience working in Social Services, directly with individuals, preferably in a healthcare setting.
• High level of professionalism and ability to maintain confidentiality.
