Job summary

Otterbein SeniorLife in Ohio is seeking a Social Services Director for Rehabilitation Services to lead resident advocacy and coordinate psychosocial care across memory care and other units.

You will collaborate with residents, families, and care teams to ensure quality of life, discharge planning, and access to community resources, while guiding regulatory compliance and staff education.

Qualifications

• Bachelor’s degree from an approved school of social work
• Current Ohio Licensed Social Worker (LSW) license
• Knowledge of resident rights, discharge planning, psychosocial assessments, care planning, MDS documentation, Medicare, Medicaid, EMR systems, and standard business tools.

Responsibilities

• Coordinate the resident admission process, including assessments, social histories, psychosocial evaluations, and required documentation.
• Provide counseling, education, advocacy, and support to residents and families to promote quality of life, adjustment, and resident-centered care.
• Serve as a resident advocate by upholding resident rights, dignity, confidentiality, self-determination, and access to appropriate services and resources.
• Participate in interdisciplinary care planning meetings and complete MDS, care plans, progress notes, and related documentation in a timely and accurate manner.
• Develop, implement, and coordinate discharge plans in collaboration with residents, families, care teams, community agencies, and service providers.
• Maintain relationships with community resources and coordinate referrals to support residents’ social, financial, medical, and psychosocial needs.
• Lead and oversee Social Services functions, including regulatory compliance, policy development, quality improvement initiatives, staff education, and departmental operations.
• Facilitate resident advocacy and communication efforts through Resident Council participation, committee involvement, grievance resolution, staff consultation, and collaboration with residents, families, and interdisciplinary teams.

The Social Services Director for Rehabilitation Services reports to the Administrator. The Social Services Director will work closely with the residents and family members of those residing on the memory care support unit. The Social Services Director identifies and facilitates social and emotional needs of the residents, families, and responsible party. Assists them with the adjustment to the psychosocial and socio-economic impact of illness; assists the continuity of care by linking patients and families with community resources to resolve personal, financial, and environmental difficulties related to illness that will maintain or improve their ability to manage their everyday physical, mental, and psychosocial needs.

Education and Experience:

• Required: Bachelor’s degree from an approved school of social work

• Preferred: Minimum of two years of experience in long‑term care, nursing home, healthcare, or geriatric social services
• Dementia training or ability to complete required dementia training in accordance with facility and regulatory expectations.
• Knowledge of resident rights, discharge planning, psychosocial assessments, care planning, MDS documentation, Medicare, Medicaid, community resources, electronic medical record systems, and standard business tools.
