Qualifikationen

• Bachelor’s degree required.
• Experience as a Social Services Director in a skilled nursing facility or long-term care setting required.
• Strong knowledge of discharge planning, psychosocial assessments, care planning, resident rights, and skilled nursing facility operations.
• Strong written and verbal communication skills.
• Excellent organization, documentation, and follow-through.
• Ability to work collaboratively with residents, families, staff, and outside providers.
• Ability to manage multiple priorities in a fast-paced healthcare environment.

Aufgaben

• Oversee and coordinate social services for residents.
• Participate in interdisciplinary care plan meetings with Nursing, Rehabilitation, Activities, Dietary, and others.
• Coordinate discharge planning and assist with community resources and placement.
• Support residents and families with adjustment to placement and changes in condition.
• Assist with resident grievances and ensure timely resolution.
• Maintain accurate social services documentation per policies and regulatory requirements.
• Support residents’ rights, dignity, choice, and quality of life.
• Communicate effectively with residents, families, physicians, hospitals, and community agencies.
• Ensure the Social Services Department remains compliant with federal and CA SNF requirements.
