Job summary

Monarch Healthcare Management is seeking a Social Services Director to plan, organize, and direct our Social Services Department in accordance with federal, state, and local regulations. This role manages admissions/discharges, referrals, screenings, tours, financial assistance, and discharge planning while supervising staff and advocating for residents.

The ideal candidate will hold a Minnesota social work license, demonstrate strong communication and leadership abilities, and support residents

Qualifications

• Must hold a valid Minnesota license to practice social work.
• Must understand the psychosocial and emotional needs of residents.
• Requires strong communication, organization, management, and leadership skills.
• Must pass a criminal background check and preferably have experience in healthcare or long-term care settings

Responsibilities

• Plan, organize, and direct all Social Services Department operations per regulations and policies.
• Manage admissions and discharge processes, referrals, screenings, tours, and financial assistance.
• Provide resident assessments, care planning, supervision of social services staff.

Social Services Director

Position Summary:

The Social Service Director is responsible for planning, organizing, and directing all Social Services Department operations in accordance with federal, state, and local regulations, facility policies, and the Administrator’s guidance. This role manages the full admissions and discharge process including referrals, screenings, tours, financial assistance, and individualized discharge planning. All while providing resident assessments, care planning, and supervision of social services staff. The Director advocates for residents by offering emotional and psychosocial support, facilitating adjustment to facility life, communicating needs to staff, and fostering strong family involvement. Additional duties include maintaining regulatory compliance and detailed documentation, participating in inspections and leadership meetings, contributing to policy development and strategic decisions, educating stakeholders on rights and safety, and proactively preventing or resolving escalated situations with the interdisciplinary team.

Qualifications:

• Licensure Requirement: Must hold a valid Minnesota license to practice social work.

• Resident Care Insight: Must understand the psychosocial and emotional needs of residents.

• Key Competencies: Requires strong communication, organization, management, and leadership skills.
