Job summary

Sapphire Health Services, LLC is seeking a Social Services Director for our Skilled Nursing facility in Oregon. The role focuses on counseling residents and families, coordinating social services, and promoting residents’ potential during their stay in collaboration with physicians for discharge planning.

You will conduct psychosocial assessments, coordinate admissions and discharges, and connect residents with dental, optometry, audiology, psychology and other community health resources while

Qualifications

• Bachelor's Degree in Social Services or a related field.
• Skilled Nursing facility experience preferred.

Responsibilities

• Interview all new residents to assess psychosocial needs.
• Coordinate admission and discharge planning.
• Counsel residents individually or with family/support groups.
• Refer residents to dental, optometry, audiology, podiatry, psychology, psychiatry and other community health resources.
• Compile records and prepare reports such as theft and loss.
• Coordinate with nursing to meet residents' optical, dental, and audiological needs and arrange transportation.
• Document social service interventions in residents' charts.
• Attend resident care meetings, rehabilitation and utilization review, QA and department head meetings.
• Complete appropriate sections of MDS timely.
• Perform other duties as assigned.

JOB SUMMARY:

A Social Services Director working with our Skilled Nursing facility will counsel, and aide residents and families requiring assistance. This position acts to meet those needs through individual and group social services programming. This position also promotes residents' potential during their stay and collaborates with physician and resident in preparation for discharge.

RESPONSIBILITIES:

• Interviewsall new residents to assess their psychosocial needs
• Performs admission and discharge planning coordination
• Counsel's residents individually, or with family/support groups
• Refers residents to needed services such as Dental, Optometry, Audiology, Podiatry, Psychology, and Psychiatry and other community health resources
• The Social Services Director will compile records and prepare reports such as theft and loss
• Coordinates with nursing department to meet the residents' optical, dental, and audiological needs and arranges transportation
• Prepares timely documentation of social service interventions and assessments in resident's chart
• Attends resident care meetings, rehabilitation and utilization review, quality assurance, and department head meetings
• Complete appropriate sections of MDS timely
• Other Duties as Assigned

REQUIREMENTS:

• Bachelor's Degree in Social Services or similar field
• Skilled Nursing experiencepreferred
