Your Role:

• Manage and oversee the Social Services Department to meet the emotional and psychosocial needs of residents

• Conduct interviews, assessments, and participate in the MDS process

• Develop individualized care plans and ensure proper documentation

• Coordinate care conferences, family communications, and discharge planning

• Foster partnerships with healthcare providers and community organizations

• Maintain compliance with all regulations and facility standards

• Serve as a key member of the leadership team and collaborate across departments

Who We’re Looking For:
