Palm Garden of Mattoon is seeking a compassionate Social Services Director to lead our social services department and ensure residents receive the highest level of care and support.

You will develop and implement social services programs, collaborate with nursing and therapy teams, and oversee assessments and care plans to promote resident well-being and dignity.

Competenze

• 1 year of experience in a social services role preferred.
• Bachelor's degree in Social Work, Gerontology, or related field.
• Excellent communication and interpersonal skills.
• Ability to work with residents, families, and other healthcare professionals.

About The Role

We are seeking a compassionate and experienced Social Services Director to join our team at Palm Garden of Mattoon. As a Social Services Director, you will be responsible for leading our social services department and ensuring that our residents receive the highest level of care and support. This is a FULL TIME opportunity that offers the chance to make a meaningful difference in the lives of our residents.

Responsibilities

• Develop and implement social services programs that meet the needs of our residents, including dementia care, hospice, and palliative care
• Collaborate with other departments, including nursing, therapy, and dietary, to ensure that our residents receive comprehensive care
• Work closely with direct care staff to support our resident's social service needs
• Complete social service assessments timely and attend quarterly care plan meetings
• Ensure compliance with regulatory requirements and industry standards

Requirements

• 1 year of experience in a social services role, preferably in a skilled nursing home setting is preferred but we will train the right candidate
• High school diploma required. Bachelor's degree in Social Work, Gerontology, or a related field (or equivalent experience)
• Excellent communication and interpersonal skills, with the ability to work effectively with residents, families, and other healthcare professionals
• Strong problem-solving and organizational skills, with the ability to prioritize tasks and manage multiple responsibilities
