Job summary

Palm Garden of Mattoon is seeking a compassionate and experienced Social Services Director to lead the social services department and ensure that our residents receive the highest level of care and support. Qualifications

• 1 year of experience in a social services role, preferably in a skilled nursing home setting; willingness to train the right candidate
• Bachelor's degree in Social Work, Gerontology, or a related field (or equivalent experience)
• Excellent communication and interpersonal skills, with the ability to work effectively with residents, families, and other healthcare professionals
• Strong problem-solving and organizational skills, with the ability to prioritize tasks and manage multiple responsibilities

Responsibilities

• Develop and implement social services programs to meet residents' needs, including dementia care, hospice, and palliative care
• Collaborate with nursing, therapy, and dietary teams to ensure comprehensive resident care
• Work closely with direct care staff to support residents' social service needs
• Complete social service assessments timely and attend quarterly care plan meetings
• Ensure compliance with regulatory requirements and industry standards
