Palm Garden is seeking a Director of Social Services (Social Worker) to lead clinical social work within our post-acute care setting in Florida. The role focuses on coordinating with families, nurses, therapists and providers to support residents' needs per care plans, and guiding discharge planning and transitions.

Qualifications

• 1-3 years of social services experience in a healthcare setting
