Job summary

Brookshire Post Acute in Denver, CO is seeking a Social Services Director to lead psychosocial care for residents in a full-time capacity. You will play a pivotal role on a caring team, ensuring residents and families receive compassionate, person-centered support.

You will oversee social services, coordinate with care staff, and guide residents through complex needs while promoting a supportive environment.

Qualifications

• Bachelor's Degree in Social Work or Human Services; MSW preferred.
• At least 2 years of supervised social work in a healthcare setting.
• Experience in post-acute or skilled nursing is a plus.

Responsibilities

• Direct and provide social services to meet residents' psychosocial needs.
• Collaborate with care teams to support residents and families.
• Ensure compliance with regulatory and quality standards.
• Lead and mentor social services staff and coordinate with interdisciplinary teams.

Successful candidates:

• Bachelor's Degree in Social Work or in Human Services - MSW preferred
• 2 years of supervised social work experience in a health care setting working directly with individuals.
• Experience in a post-acute or skilled nursing facility a plus!

Rate : $31.25-$36.06/hour
