Job summary

Signature HealthCARE seeks a Social Services Director for our Clarksville facility. You will identify residents’ social, emotional, and psychological needs, and guide ongoing development during their stay while assisting discharge planning.

You will support families, ensure compliant documentation, and participate in care and discharge planning with the care team. The role requires a Bachelor's or Master's in Social Work or Psychology and 3+ years in Social Services, ideally in healthcare, with

• 3+ years of experience in Social Services, directly with individuals, preferably in healthcare.
• High level of professionalism and ability to maintain confidentiality.

Responsibilities

• Identify and address each resident’s social, emotional and psychological needs during stay and assist in discharge planning.
• Assist residents and families with social, emotional, and family issues.
• Complete documentation timely and accurately to comply with facility, state and federal policies.
• Participate in resident care planning.
• Contribute to discharge planning and transition preparation.
• Serve as a reliable information source for residents and families.

Overview

As the Social Services Director you will identify and provide for each resident’s social, emotional and psychological needs, and the continuing development of the resident’s full potential during his/her stay at the facility and to assist in the planning for his/her discharge.

How You Will Make a Difference

• Assist residents and their families with social, emotional, and family issues.
• Provide timely and accurate completion of documentation needed to comply with Facility, State, and Federal policies.
• Participate in resident care planning.
• Participate in the development of Discharge planning and prepare resident for the transition.
• Serve as a general source of information for residents and their families.

What You Need To Make a Difference

• Bachelor’s or Masters Degree in Social Work or Psychology.
• 3+ Years Experience working in Social Services, directly with individuals, preferably in a healthcare setting.
• High level of professionalism and ability to maintain confidentiality.
