Job summary

South Shore Rehabilitation is seeking a dedicated Social Services Director to lead the Social Services team in a Skilled Nursing facility. The role focuses on monitoring residents’ psychosocial needs and coordinating discharge planning with therapy, nursing, and other departments to ensure high-quality care.

Responsibilities include developing care plans, participating in plan meetings, completing admissions/discharges documentation, and navigating discharge processes with families and community

Qualifications

• Bachelor's degree in Social Work (BSW) or related field.
• 2-3 years of experience in social services/discharge planning within a long-term care or skilled nursing facility.
• Strong knowledge of Medicare, Medicaid, and insurance processes.
• Excellent communication and leadership skills.
• Ability to work as part of an interdisciplinary team.

• Develop ongoing care plans based on residents’ needs identified in assessments.
• Participate in care plan meetings to ensure coordinated approach.
• Complete required documentation upon resident admission, discharge, and transfer.
• Implement healthcare directives and identify end-of-life care opportunities.
• Assess residents' discharge potential with the interdisciplinary team.
• Plan and coordinate discharge services with residents, families, and community resources.
• Assist residents and families in navigating the discharge process and ensuring a smooth transition.

Social Services Director Requirements:
