Job summary

Yellowstone River Nursing and Rehab seeks a dedicated Social Services Director to oversee admissions, discharges, and the plan of care for residents. You will coordinate family conferences, advocate for participant rights, and liaise between participants, families, and the center.

The role emphasizes geriatric social work within a Skilled Nursing Facility setting, with a focus on community relations and ensuring high-quality resident care. This is a full-time, on-site position in Billings, MT.

Qualifications

• Must have graduated from an accredited school and hold a valid MSW or LMSW in good standing.
• Experience with geriatric patients or in a Skilled Nursing Facility is a major plus.

Responsibilities

• Facilitate admissions and discharges in consultation with the Administrator.
• Provide family conferences and act as liaison among participant, family and Center.
• Promote participant involvement in plan of care and assess for signs of mental illness or dementia.
• Coordinate additional support services not available at the Center.
• Advocate for participants and ensure discharge planning and follow-up.

The primary purpose of this position is to ensure the highest quality of resident care available, support staff and establish a positive reputation in the community while delivering on the Eduro HealthCare Values of wellness, compassion, customer experience and company results.

Social Services Director Responsibilities:

In consultation with the Administrator, facilitating all admissions and discharges;

• Ensure development and regular review of appropriate discharge plans in conjunction with overall plan of care.
• Provide family conferences and serve as a support and liaison between the participant, family, and Center.
• Facilitate participant’s adaptation to the adult day health care program.
• Promote and facilitate participant active involvement in plan of care as appropriate.
• Assess participants for signs of mental illness and/or dementia and make appropriate referrals.
• Arranging and coordinating additional support services which are not available at the Center.
• Serving as a participant advocate by safeguarding their civil and human rights.
• Discharge planning, assistance in transition and follow-up.
• Participating in social service quality assurance evaluation.
• Follow-up of hospitalized participants.
• Facilitation of support groups to family and participants through individual or group counseling.
• Conduct support groups or facilitate participant or family/caregiver participation in support groups.
• Assist program staff in adapting to changes in participant’s behavior.

Social Services Director Requirements:

• Must have graduated from an accredited school and hold a valid MSW or LMSW in good standing.
• Previous experience in Social Work.
• Experience with geriatric patients or in a Skilled Nursing Facility is a major plus!
• Successful completion of a training and competency evaluation program required.
• Completion of orientation required by company policy.
• Health screening and TB test (if required) within two weeks of employment.
• Mandatory drug test within 90 days of hire.
• Must be free of criminal activity proven by a criminal background check.
• Show within three (3) days of hire satisfactory evidence of identity and eligibility for employment.
