Job summary

Avon Health and Rehabilitation in Avon, IN is seeking a Social Services Director to lead the department and ensure residents' social needs are met in compliance with federal, state and local standards.

You will interview residents and families, develop care plans, coordinate with care teams, arrange counseling as needed, and chair the Behavior Management and Antipsychotic Committee to support quality of life. This role offers competitive benefits and opportunities for professional growth.

Qualifications

• Bachelor’s Degree in Social Work (BSW, MSW, CSW’s preferred) or a Bachelor’s in a human services field such as sociology, special education, rehabilitation counseling or psychology.
• Minimum of one year of supervised social work experience in a health care setting with patients.
• Social Services Designee form from the state of employment if the individual does not possess a Bachelor’s Degree.
• Thorough understanding of the principles of best Social Services practices.

Responsibilities

• Oversight of the facility’s Social Services Department and ensuring that the needs of the residents are met on an individual basis.
• Interviewing resident/families to obtain social history and involving the resident/family in planning objectives, goals and social services programs.
• Assisting in the development of a written plan of care for each resident that identifies the problems/needs, the goals to be accomplished, and the recommended approach for each.
• Interpreting social, psychological and emotional needs of the resident to the medical staff, attending physician and other resident care team members.
• Arranging psychological care and evaluation services as needed.
• Assisting in the scheduling of care plans and assessments to be presented and discussed at each committee meeting as needed.
• Ensuring that all social services personnel are aware of the care plan.
• Reviewing nurses notes to determine if the care plan is being followed.
• Chairing the Behavior Management and Antipsychotic Committee.

• Must possess a Bachelor’s Degree from an approved school of Social Work (BSW, MSW, CSW’s are preferred) or a Bachelor’s in a human services field including but not limited to sociology, special education, rehabilitation counseling and psychology
