Job summary

Bethel Lutheran Nursing & Rehabilitation Center is seeking a Full-Time Social Service Designee to provide resident advocacy, conduct social assessments, and support directives and rights coordination. The role also involves participating in MDS processes and updating care plans and records. Orientation is provided upon employment.

Background check and drug screen are required. The center is a Christian nonprofit organization dedicated to serving the neighborhood and is an equal opportunity

Qualifications

• Experience with social services in a care setting.
• Ability to complete social assessments.
• Ability to maintain patient records and care plans.

Responsibilities

• Advocate for residents and their needs.
• Complete social assessments and review directives, resident rights, and ombudsman program with residents and families.
• Participate in MDS and other required assessments; update medical records and progress notes.
• Coordinate with residents, families and community resources for referrals and solutions.
• Cross-train for admissions process with the admissions lead.

BLNRC is seeking a Full-Time Social Service Designee.Work schedule would be Monday-Friday, 8:00am-4:30pm.All employees are required to complete orientation program upon employment.

Duties include but are not limited to, advocating for residents and their needs, completing social assessments, reviewing advance directive,resident rights, and ombudsman program with residents and their families, participate in MDS and other required assessments, updating medical records, progress notes and care plans as needed/required, and working with residents, families and community resources to assist inreferral servicesolutions inside or outside of the facility to meet resident needs.

Candidate will cross train for admissions process with current admission lead.
