Job summary

Parma Care Center in Parma, Ohio, seeks a compassionate Social Services Designee to support residents’ emotional and social adjustment, advocate for resident rights, and coordinate with families and community resources to improve overall well-being.

You will conduct psychosocial assessments, develop individualized care plans, participate in care conferences, and assist with transitions and MDS-related activities to ensure seamless resident care.

Qualifications

• Must meet state requirements for social service designation in long-term care.
• Minimum 2 years of long-term care experience preferred.
• Strong communication and organizational skills.
• Experience in public relations and in-service training is a plus.
