Job summary

Parma Care Center in Parma, OH seeks a compassionate Social Services Designee to support residents in their emotional and social adjustment to long‑term care. You will advocate for resident rights, conduct psychosocial assessments, and coordinate with families and community resources.

The role requires state designation for social services in long‑term care, strong communication and organizational skills, and the ability to participate in care conferences and transitions.

Qualifications

This role is vital in advocating for resident rights and enhancing their psychosocial well-being through assessments, care planning, and coordination with families and community resources.

Qualifications:
