CommuniCare Health is seeking a Social Services Designee at Greenfield Healthcare Center in Greenfield, IN. The role focuses on residents' psychosocial well-being, advocacy, and coordinating with care staff to ensure needs are met.

Qualifications

Responsibilities

• Develop and maintain rapport with direct caregivers to support programs/services for residents' psychosocial well-being.
• Report grievances and complaints to the Director of Social Services with written documentation.
• Advocate for residents, ensuring needs are met with appropriate dress and appearance.
• Notify RN Charge Nurse of any changes in residents' condition.
• Provide information to improve services and activities for residents.
• Collaborate with activity department to involve family and guests.

E. D. equivalent* 3-4 years previous social services experience in a long term care environment.## **YOUR MISSION****As Social Services Designee, you will:*** Develop and maintain a good working rapport with other direct caregivers to assure their participation in and support of all programs/services to benefit the residents’ psychosocial well being.* Report all grievances and complaints and make necessary oral/written reports to the Director of Social Services.* Serve as the resident’s advocate at all times working in harmony with all direct care giving staff to assure that the resident’s needs are being met at all times (i.e. appropriate dress and clean appearance).
