Job summary

Willow Tree Healthcare Center, a member of the CommuniCare Family, is seeking a Social Services Designee in Charles Town, WV for a full-time role. LSW preferred; a high school diploma or GED is required, with prior social services experience in a long-term care environment.

You will advocate for residents, coordinate with direct caregivers, report grievances to the Director of Social Services, and help meet residents' psychosocial needs with confidentiality and professionalism.

Responsibilities

• Develop and maintain a good working rapport with direct caregivers to assure participation in programs/services benefiting residents' psychosocial well-being.
• Report grievances and complaints and make necessary oral/written reports to the Director of Social Services.
• Serve as the resident’s advocate, ensure needs are met, and monitor changes and report to the RN Charge Nurse.
• Provide information to the Director of Social Services to help solve residents' problems and meet their needs.
• Improve quality of services and activities, increasing opportunities and choices for residents.
• Work with the activity department to involve family members and guests in activities where possible.
• Maintain confidentiality of all resident/family information.
• Perform general social services duties.
