Job summary

Care Initiatives seeks a Social Services Coordinator for Westwood Specialty Care in Sioux City, IA. You will assess residents’ psychosocial needs, complete social histories, and support discharge planning to ensure smooth transitions.

You will provide counseling to residents and families, coordinate referrals with community resources, and collaborate with the care team to improve quality of life and outcomes across the facility.

Qualifications

• Bachelor’s degree in social work or a related health and human services field.
• Ability to obtain a Licensed Social Worker (LSW).
• Minimum of one year supervised social work experience in a health care setting.
• Strong interpersonal and communication skills with residents and families.

Responsibilities

• Assess residents’ psychosocial needs and complete social histories.
• Complete assigned MDS sections accurately and on time.
• Document care plans, progress, and follow-up per regulations.
• Provide counseling and emotional support to residents and families.
• Support residents and families with adjustment, rights, and orientation.
• Assist with discharge planning and care transitions.
• Coordinate referrals and liaise with community resources.
• Partner with the care team to enhance quality of life and outcomes.
• Plan and support resident, family, and community engagement activities.
• Promote facility programs and referral partnerships.
