Job summary

Care Initiatives in Iowa seeks a compassionate Social Services Coordinator to support residents’ psychosocial well-being and coordinate discharge planning within our skilled nursing communities across Iowa.

You will assess needs, document care plans, provide counseling, and collaborate with the care team to enhance quality of life and transitions for residents and families. A bachelor’s in social work or related field and ability to obtain LSW are required.

Qualifications

• Bachelor’s degree in social work or related field.
• Ability to obtain an LSW.
• 1 year of supervised social work experience in a health care setting.

Responsibilities

• Assess residents’ psychosocial needs and complete social histories.
• Complete assigned MDS sections accurately and on time.
• Document care plans, progress, and follow-up per regulations.
• Provide counseling and emotional support to residents and families.
• Support residents and families with adjustment, rights, and orientation.
• Assist with discharge planning and care transitions.
• Coordinate referrals and act as a liaison with community resources.
• Partner with the care team to enhance quality of life and outcomes.
• Plan and support resident, family, and community engagement activities.
• Promote facility programs and services to referral partners.
