Responsibilities

• Provide assistance with activities, social, emotional and economic concerns of residents and families.
• Utilize knowledge of age-related resident care protocols for adults and geriatrics.
• Work with interdisciplinary team to achieve sound and timely outcomes; may formulate care plans and facilitate care conferences.
• Investigate grievances and align resources while maintaining teamwork and communication.
• Demonstrates ability to establish rapport with residents, families and the healthcare team.
