Job summary

Eden East is seeking a Social Services Coordinator to support the social services team in resident care, interviews, and discharge planning. You will document activities and coordinate with interdisciplinary staff to ensure coordinated care.

The role requires communication skills, professionalism, and a PA social work license where possible. Work is performed on-site with opportunities to grow within the organization.

Qualifications

• Bachelor’s degree in Social Work or related Human Services field.
• Licensed Social Work certificate in Pennsylvania is a big plus.
• Experience in health care social work preferred but not required.

Responsibilities

• Conduct resident interviews to assess social, emotional, and financial needs.
• Assist in discharge planning coordinating with families and providers.
• Document interviews and care conferences per facility policies.
• Communicate with nurses, social workers, and therapists for care planning.
• Lead discharge planning with external resources for safe transitions.
• Provide information on Medicare, Medicaid and other assistance programs.
