Care Initiatives is seeking a Social Services Coordinator to assess psychosocial needs, complete MDS sections, and document care plans across multiple nursing communities in Iowa. You will support residents, families, and transitions with empathy and professionalism.

Qualifications

• Bachelor’s degree in social work, gerontology, or related field.
• Ability to obtain a Licensed Social Worker (LSW).
• Minimum of one year of supervised social work experience in a health care setting.
• Interpersonal and communication skills to work effectively with others.
• Strong commitment to helping others with empathy, respect, patience, and discretion.

Qualifications

• A bachelor’s degree in social work, gerontology, or a related field within health and human services including, but not limited to, sociology, special education, rehabilitation, counseling and psychology.
• Ability to obtain a Licensed Social Worker (LSW).
• Minimum of one year of supervised social work experience in a health care setting working directly with individuals.
• Interpersonal and communication skills that identify the ability to positively and effectively communicate with others while completing work accurately and timely.
• A strong commitment to helping others and consistently treating them with empathy, respect, patience, and discretion.
