Novant Health is seeking a Manager of Case Management to oversee operations, quality, and productivity across the case management process. You will coordinate with facility and service line leaders to ensure continuity of care and connect with community resources to sustain relationships.

The role requires 3 years in case management, RN licensure if applicable, and a leadership background; a Graduate Degree is preferred and knowledge of budgeting and staffing is essential.

Qualificações

• Education requirements: 4 Year / Bachelors Degree in Nursing or Social Work or related healthcare field; Graduate Degree preferred.
• Three years experience in Case Management or related field; Leadership experience preferred.
• RN licensure if applicable; Certification in Case Management preferred.
• Extensive knowledge of adult learning, teaching methods/models, budgeting, staffing, payroll and purchasing processes; strong communication and delegation skills.

What You'll Do

The Manager of Case Management is responsible for oversight and management, operations and ongoing quality, productivity and efficiency for case management processes. Coordinates with facility and service line leaders to ensure continuity of care throughout the continuum. Connects with community resources and leaders to establish and maintain relationships.

What We're Looking For

• Education:
• 4 Year / Bachelors Degree Bachelor's Degree in Nursing or Bachelor's Degree in Social Work accredited by the Council on Social Work Education or Bachelor's Degree in related healthcare field, required.
• Graduate Degree, preferred.
• Experience:
• Three years experience in Case Management or related experience, required.
• Leadership experience, preferred.
• Licensure/Certification:
• If RN, current RN licensure in appropriate state, required.
• Certification in Case Mgmt. or related field, preferred.
• Additional Skills (required):
• Current and future trends/practice in area of responsibility; regulatory and governing standards, policies and procedures; information systems and software used in area of responsibility; equipment used in performing assigned duties; application of safety and infection control policies and procedures; skills in interviewing and selection process; understanding of budgeting, staffing, payroll and purchasing processes; implementation of competency, feedback, development and progressive discipline processes; teambuilding; conflict management, and interpersonal relationship skills; principles of adult learning and teaching methods/models; decision making, project leadership and consultation; delegation and communication skills; understanding of customer needs and expectations and knowledge for improvement to exceed customer expectations. Develops and continuously improves discharge planning core processes. 24/7 department accountability.

• Knowledge/Competence of Microsoft office products and keyboarding.
