Job summary

Parkwood Health and Rehabilitation in Louisville, KY is seeking a Social Services Assistant to support residents and families in a compassionate, resident-centered setting. This full-time role runs Monday through Friday with two shift options, 8:00–4:30 or 9:00–5:00, and offers competitive hourly pay and robust benefits.

You will collaborate with an interdisciplinary team to address social and emotional needs, assist with documentation, and coordinate resources, appointments, and discharge

Qualifications

• Strong communication and interpersonal skills.
• Compassionate and patient approach with residents and families.

• Ability to work effectively in an interdisciplinary team.
• Proficiency with basic computer applications and documentation.
• High school diploma or equivalent; further education in social services or related field preferred.

Responsibilities

• Assist the Social Services department with supporting residents and their families.
• Help residents adjust to the skilled nursing environment and address social and emotional needs.
• Assist with completing and maintaining resident documentation and records.
• Communicate with residents, families, staff, and outside resources as needed.
• Assist with care plan meetings and follow-up activities.
• Help coordinate appointments, referrals, and community resources.
• Assist with discharge planning and transition-related activities.
• Support resident rights, dignity, privacy, and quality of life.
• Participate in interdisciplinary team meetings and facility activities.
• Maintain confidentiality and follow all facility policies and applicable regulations.
