You will assist with psychosocial support, coordinate community resources, and help ensure regulatory compliance while maintaining resident rights and safety. Join a dedicated team focused on dignity, comfort, and well-being.

Qualifications

• College degree preferred.
• Long-term care experience required.
• Ability to make independent decisions when appropriate.
• Knowledge of post-acute care regulations and guidelines.
• Ability to implement departmental programs and policies.
• Competence in patient rights, safety, and sanitation.
• Professional integrity and compliance training.

Responsibilities

• Support residents' psychosocial and practical needs.
• Coordinate access to community resources and services.
• Help implement departmental goals and procedures.
• Communicate with residents, families, and care team for care planning.
• Assist with documentation and records in Social Services.
• Support compliance with resident rights and safety standards.
