Job summary

InnovAge is seeking a Social Worker Assistant to support Social Work staff with administrative duties under supervision. The role focuses on non-clinical tasks to help participants access services in the Newport News area, aligning with InnovAge policies.

Responsibilities include assisting with Medicaid/recertification, researching community resources, updating EPIC records, and coordinating outreach to ensure timely supports under the center's program.

Qualifications

• Community Health Worker training or certification is required.
• Associate’s degree in healthcare, social services, or related field is required; bachelor’s preferred.
• Must have a valid driver's license and transportation.

Responsibilities

• Confirm delivery of essential DME, goods, services to support participants.
• Assist participants with Medicaid/Medicare applications or recertifications.
• Research, identify and catalogue community resources and programs for referrals.
• Update participant demographic information and contacts in InnovAge systems as required.
• Travel may be required within the Center service area.

Education

Community Health Worker training or certification

Associate’s degree in healthcare or social services

Bachelor’s degree in Social Work or Health Care Administration
