Job summary

Longwood Manor Convalescent Hospital is seeking a Social Services Assistant to support the Social Services Director in meeting residents' emotional and social needs with compassionate, person-centered care.

You will assist in planning, coordinating, and communicating with administration, nursing staff, and external agencies to ensure appropriate admissions and ongoing service delivery. A background in social work and strong interpersonal skills are essential for success.

Qualifications

• Proven work experience as a social worker.
• Fully vaccinated against COVID-19 including Booster shots.
• BS degree in social work (BSW) preferred, psychology or sociology preferred, but not a requirement.
• Working knowledge of social theories and practices.
• Social perceptiveness and empathy.
• Ability to build and maintain professional helping relationships.
• Ability to relate and communicate with diverse population and groups.
• Resilience along with ability to assess situations.

Responsibilities

• Assists in the planning, developing, organizing, implementing, evaluating, and directing of the social services programs of the facility.
• Meet with administration, medical and nursing staff, and other related departments in planning social services.
• Develop and maintain a good working rapport with inter-department personnel, other departments within the facility, and outside community health, welfare, and social agencies, to assure that social service programs can be properly maintained to meet the needs of the patient.
• Assist in the development, administering, and coordinating of department policies and procedures.
• Assists with the admissions process; assists in the determination of admissions appropriateness, compile assessment information on incoming residents’ mental and physical condition in addition to financial status, and completes necessary forms.
• Other duties may be assigned.

Depending on a number of factors, including, but not limited to location, regularly scheduled work shift, knowledge, skills, experience, and expertise.
