Job summary

Tuskawilla-Nursing- in the United States is seeking a Social Services professional to support admissions and discharge processes and to work with families and residents. The role involves coordinating care plans, tours, and room changes while ensuring policy adherence and confidential handling of information.

You will back up the Director of Social Services, help resolve concerns, and ensure documentation is accurate and timely, aligning with state and federal guidelines.

Responsibilities

• Assists in the completion of care plans and psychosocial assessments.
• Coordinates care plan meetings with families.
• Coordinates room changes and roommate notifications when necessary.
• Conducts tours of facility with potential residents/patients and their families.
• Ensures specialized equipment is ordered as indicated and seeks prior approval when required.
• Completes records and documentation in accordance with policy and guidelines.
• Helps resolve concerns of residents and/or families and participates in the Guardian Angel Program.
• Provides patient comfort by utilizing resources, transporting patients, answering call lights, and reporting observations to nursing staff.
• Serves as Primary Back-Up for the Director of Social Services when they are out on PTO, FMLA, or the Director of Admissions position is open.
• Protects patient information confidentiality and adheres to standards and policies.

Supports the Social Services Department in the admission and discharge process. Serves as the back up to the Director of Social Services. Works with social services to ensure the psychosocial needs of the residents and patients are met.

ESSENTIAL DUTIES AND RESPONSIBILITIES

• Assists in the completion of care plans and psychosocial assessments.
• Coordinates care plan meetings with families.
• Coordinates, notifies and documents room changes and roommate notifications when necessary.
• Conducts tours of facility with potential residents / patients and their family members.
• Ensures specialized equipment is ordered as indicated (reviews contract content for need to obtain prior approval from authorized payer).
• Completes records and documentation in accordance with company policy and state and federal guidelines.
• Helps resolve concerns of Resident and/or Family and participates in the Guardian Angel Program.
• Provides patient comfort by utilizing resources and materials; transporting patients; answering patients' call lights and requests; reporting observations of the patient to nursing supervisor.
• Primary Back-Up for the Director of Social Services when out on PTO, FMLA or the Director of Admissions position is open.
• Protects organization’s value by keeping patient information confidential.
• Services and protects the facility by adhering to professional standards, company policies and procedures, federal, state, and local requirements, and standards.
