Genesis in Decatur, Alabama, is seeking a social services professional to collaborate with patients, residents and families, providing support and resource information. The role supports transitions and documents accordingly, under the direction of a social worker.

You will participate in care planning, respond to concerns, and help coordinate transfers within the nursing center. Requires an associate degree in human services and state certification as required.

Qualifications

• Associate degree in human services with a minimum of two years' experience in long-term care is required.
• Must possess any certification required by state regulations.

Responsibilities

• Collaborate with patients/residents and their families to provide support and resource information.
• Respond to issues identified by patients/residents and their families.
• Facilitate patient/resident transfer within the nursing center to ensure a seamless transition and patient/resident adjustment and document accordingly.
