Job summary

Diversicare in Amory, MS is seeking a Social Services Associate to assist with admissions, Medicaid eligibility, and care coordination. You will work with residents, families, and healthcare professionals to ensure smooth transitions and appropriate support.

Qualified candidates have a high school diploma or equivalent; a BA in Social Work or Human Services is preferred, with experience in healthcare and ICP Medicaid processes. This is a full-time role with comprehensive benefits.

Qualifications

  • Minimum: high school diploma or equivalent.
  • Bachelor’s Degree in Social Work or Human Services preferred.
  • One year of supervised social work experience in health care recommended.
  • ICP Medicaid qualifications knowledge is a plus.

Responsibilities

  • Assist in pre-admission and admission processes, including tours and paperwork.
  • Interview clients and families to determine Medicaid eligibility and financial status.
  • Assist with applying for ICP Medicaid approval and annual reviews.
  • Support community and health care discussions and update plans of care.
  • Maintain communication with residents, families, and staff.

Responsibilities
  • Assist in pre-admission and admission process with customers, as needed, as back-up to Admissions Department personnel. Duties to include providing tours,signing of paperwork, etc.
  • Interview clients and families to determine financial status and need for Medicaidassistance.
  • Assist families in applying for clients’ Medicaid (ICP) approval. Attend annualreview for Medicaid eligibility if families unable to attend.
  • Assist facility bookkeeper in tracking and reporting status of Medicaid Applications/Approvals.
  • Assist Social Services Director, as needed, with interviewing residents, familiesto obtain relevant past and present social histories, including past behaviors, adjustments, and lifestyles.
  • Documentsprogress and updates plans of care at interdisciplinary conference, asneeded.
  • Assist residents and families with adjustment to admission to facility by providing one-on-one counseling and/or group discussions.
  • HelpSocial Services Directory with planning of post discharge services andfollow-up care.
  • Maintaineffective communication with residents, families, health careprofessionals and facility staff.
  • Assistresidents and families in any situation, as needed.
  • This person may have access to protected health information in the patient’sfinancial and medical records to conduct job duties.
Qualifications
  • Minimumof High School diploma or equivalent. Bachelor’s Degree in Social Work or Human Services is preferred.
  • One yearof supervised social work experience in a health care setting, working directly with individuals.
  • Experience or knowledge of Institutional Medicaid Program (ICP) qualifications andapplication process preferred.
  • Ability to work with a variety of people in situations varying from community andhealth care settings.
  • Able to make decisions concerning procedures based on evaluations and interpretations of clients’ case histories
