Job summary

Ahava Healthcare in Athens, TN seeks a Social Services and Admissions Coordinator for a Skilled Nursing Facility. You will assist the Admissions Director and teams to grow the facility's census, manage admissions, and oversee the Social Services Department.

The role emphasizes coordination of resident care plans, discharge planning, and HIPAA-compliant handling of information, with a focus on providing compassionate, high-quality placement assistance for residents and families.

Qualifications

• Bachelor’s degree or experience in social work or healthcare admissions.
• Nurse experience as an LPN preferred (Tennessee license).

• Assist Admissions Director, Clinical Liaison, and Marketing Director to grow the facility's census.
• Manage the admission process including Room Control, follow-up, and occupancy levels; complete admissions paperwork.
• Oversee development, implementation, and evaluation of the Social Services Department.
• Assist residents and families with medically related services, grievances, or placement needs.
• Complete assessments and participate in comprehensive care planning.
• Facilitate discharge transitions and transportation for appointments.
• Maintain confidentiality of protected health information.

Social Services and Admissions Coordinator – Skilled Nursing Facility near Athens, Tennessee

What You’ll Do:

• Responsible for assisting the Admissions Director, Clinical Liaison, and Marketing Director in growing the facility's census.

• Manage the admission process by maintaining Room Control, timely follow up, and manage occupancy levels. Complete admissions paperwork.

• Oversee the development, implementation, supervision, and ongoing evaluation of the Social Services Department.

• Meet with and assist residents and their families with medically related services, grievances, or other placement needs to help maintain the highest quality of care.

• Complete assessments and participate in comprehensive care plans.

• Facilitate residents’ safe transition upon discharge as well as facilitate transportation for appointments or other scheduled events.

• Maintain confidentiality of protected health information, including verbal, written and electronic communications.

What We’re Looking For:

• A minimum of a bachelor’s degree or experience in social work or healthcare admissions

• Nurse experience as a LPN also preferred (current unrestricted LPN license in the state of Tennessee)
