Job summary

Green-Hills-Center-For-Rehabilitation-and-Healin seeks a Social Worker to support residents and families, coordinate care and discharge plans, and work with the finance team to ensure proper documentation. A BSW is required with at least 1 year in social work within long-term care.

Flexibility with schedule and EMR proficiency are essential to document care and discharge planning across departments.

Responsibilities

• Provide support to residents and families, coordinating needs and expectations during stay.
• Coordinate care and discharge plans with the interdisciplinary team.
• Collaborate with finance to ensure residents are educated and documents are in place.
• Arrange discharge with DME or home care to ensure safe return to the community.

SOCIAL WORKER

Responsibilities Include:

• Provides support to the residents and families in coordinating residents needs to ensure residents needs and expectations are met during their stay at (Facility) and their expectation in the future.
• Work closely with the interdisciplinary team in coordinating the care plan and discharge plan.
• Work with the finance team to ensure resident is educated and the proper documents are in place for the resident during their stay.
• Work with the resident on discharge and get necessary DME or home care in place so that the resident is safe when they return to the community.
