Job summary

Blossomsrnc is seeking a licensed social worker to support residents in a long-term care setting in North Little Rock, AR. The role focuses on social and emotional needs, care planning, and discharge coordination, with a collaborative interdisciplinary team.

Requirements include 1+ year of social work experience and a preferred MSW/BSW or BA in a related field. LSW/BSW/MSW are valued qualifications for this position.

Qualifications

• At least one year of experience in social work required.
• Experience in long-term care or sub-acute settings preferred.

Responsibilities

• Maintaining and/or improving residents' ability to manage daily physical needs and psychosocial needs.
• Identifying individual social and emotional needs.
• Developing and maintaining individualized care plans.
• Participating in admission, community re-entry, or transfer planning.
• Participating in interdisciplinary care plan meetings to address social/emotional needs.
• Collaborating with finance to ensure residents have proper documentation.
• Coordinating discharge planning and arranging DME or home care as needed.

Duties and Responsibilities

• Maintaining and/or improving resident’s ability to control everyday physical needs and mental/psychosocial needs
• Identifying individual social and emotional needs
• Developing and maintaining individualized care plans
• Participating in the planning of the resident’s admission, community re-entry or transfer to another facility
• Participating in interdisciplinary care plan meetings to ensure treatment of the social and emotional needs of the resident
• Work with the finance team to ensure resident is educated and the proper documents are in place for the resident during their stay
• Work with the resident on discharge and get necessary DME or home care in place so that the resident is safe when they return to the community

Qualifications/Requirements

• LSW, BSW or MSW preferred, or Bachelors degree in related field
• At least one year experience in Social Work field required
• Experience in long term care and/or sub-acute setting preferred
