Reliant Care Management Company is seeking a Social Service Director to lead social services for residents. You will oversee psychosocial assessments, coordinate resident visits, and collaborate with families and care teams to ensure comprehensive support.

Responsibilities include coordinating with medical providers, facilitating access to Medicaid/Medicare services, and guiding staff through documentation and care planning. A passion for elder care and teamwork is essential.

Qualifikationen

• Minimum of a Bachelor’s degree in social work or a human-services related major.
• One year in a healthcare setting as a supervised social worker who works directly with individuals.
• Ability to work within a team and respond positively to directions from the head of the Social Services Department.
• Interest in a variety of age groups and the ability to positively influence residents and families.
• Ability to communicate effectively, with good verbal and writing skills.

Aufgaben

• Provide Social Services to residents and their families under the direction of the social services director.
• Meet new residents on admission and complete an Initial Psychosocial Assessment; follow up visits.
• May complete some MDS assessments under supervision.
• Arrange purchases of needed items and ensure residents have clothing.
• Schedule visits to dental, eye, podiatry and audiology providers.
• Provide support in times of loss to residents and families.
• Assist residents in gathering documents for Medicaid, Medicare, SSI, and related services.
• Carry out other duties as directed by the Social Services Director.
