Job summary

Harmony Palos Heights is seeking a Social Services Designee to join our warm, supportive team in a skilled nursing environment. You will help meet residents' social and emotional needs and coordinate discharges to support their ongoing well-being.

Minimum requirements include a high school diploma and 2 years in a social work role, preferably in healthcare. Preference for a bachelor’s in social work or related field with 5 years of related experience.

Qualifications

• High school diploma or equivalent plus 2 years in a Social Worker role, preferably healthcare.
• Bachelor's degree in Social Work or related field preferred, 5 years of related experience desirable.
• Willing to invest in the right candidate with potential for growth.

Responsibilities

As a Social Services Designee you will assist in preparing our residents for their future. Your work will ensure our residents receive the high standard of care they have grown to expect at Harmony Palos Heights by ensuring our residents social and emotional needs are met during their stay, coordinating resident discharges and participating in resident care conferences. Our residents will depend on your knowledge, skills, and attention to detail to ensure they are comfortable and safe.

To be eligible for consideration applicants should have:

As a minimum, a high school diploma or equivalent and two (2) years of experience in a Social Worker role preferably in a healthcare setting. Prior Social Services Designee experience in a Skilled Nursing Facility, bachelor's degree in Social work or similar field and five (5) years of related experience is preferable but we are willing to invest in the right candidate!
