Job summary

Qualifications

• Five years of social services experience or equivalent.
• Bachelor's degree plus two years social services experience, or advanced degree with one year.
• Certification eligibility for Addictive Disorder Regulatory Authority (ADRA)r or successor.

Responsibilities

• Serves as Case Manager for medically complex, intermediate and skilled care, including Medicare veteran units with a caseload of veterans with multiple psychiatric, substance abuse, behavioral and medical problems.
• Escort and assist veterans to medical and other appointments.
• Admissions protocol involving financial affairs, medical records and placement on their assigned units and assist other Social Service staff.
• Investigates and reports resident complaints and incidents, recommending corrective actions and collaborating with Supervisor.
• Update veterans face sheet and represent department at monthly Residents meetings.

Join the State of Louisiana Department of Veterans Affairs at the Southeast Louisiana Veterans Home,where you'll step into a role as a Social Service Counselor 3 employee that combines social service counseling and excellence with genuine care for those who served. This position will serve as a Case Manager for skilled and intermediate care for Medicare veterans. Assist veterans and their family members through admissions and financial affairs. Assisting the Social Service staff.

Experience Requirements

• Five years of social services experience; OR
• Six years of full-time experience in any field plus two years of social services experience; OR
• A bachelor's degree plus two years of social services experience; OR
• An advanced degree in a social sciences field plus one year of social services experience.

Experience Substitution

Every 30 semester hours earned from an accredited college or university will be credited as one year of experience towards the six years of full-time work experience in any field. The maximum substitution allowed is 120 semester hours which substitutes for a maximum of four years of experience in any field.

Necessary Special Requirement

Positions that provide addictive disorder counseling or prevention services will require certification or eligibility for certification issued by the Addictive Disorder Regulatory Authority (ADRA), or its successor.

Job Duties

• Updating veterans face sheet, represents department ay monthly Residents meetings.
