Job summary

Nevada Health Centers is seeking a Social Service Coordinator to connect patients with social resources and coordinate access to medical and psychosocial programs as part of the interdisciplinary care team.

The SSC will document assessments and plans, maintain community resources knowledge, and collaborate with Behavioral Health and Primary Care providers to ensure integrated care and timely referrals. This role emphasizes compassionate, patient-centered service.

Qualifications

  • Bachelor's degree in Health and Human Services or related field.
  • Current Nevada State Social Work Licensure (LSW) required or preferred.

Responsibilities

  • Assess, plan, and coordinate access to medical and psychosocial programs.
  • Document assessments and plans according to NVHC guidelines.
  • Maintain knowledge of community resources for case management.
  • Collaborate with Behavioral Health and Primary Care teams to evaluate progress against the care plan.
  • Serve as a liaison to inform patients about integrated treatment and referrals to behavioral health.
  • Assist Behavioral Health leadership in coordinating referrals and screenings (PHQ-2, PHQ-9).
  • Build community partnerships with outside agencies.

Job description

Position Summary

The Social Service Coordinator (SSC) shall have primary responsibility for connecting patients to social resources and coordinating access to medical, psychosocial programs and social case management.

Essential Duties and Responsibilities
  • Responsible for the assessment, planning, and access coordination of medical, psychosocial programs and/or other social needs.
  • Documents assessment, plan and interactions according to NVHC established guidelines and standards
  • Maintains knowledge of community resources in order to provide for patient’s case management needs
  • Collaborates with Behavioral Health and Primary Care providers, and other members of the patient’s interdisciplinary care team, in evaluating patient progress against the care plan; reviews and communicates with patient’s revisions/updates of plan and/or goals.
  • Serves as a resource with Primary Care Providers in conducting warm-hand offs to inform patients about the integrated treatment model and the benefits of behavioral health services and a referral to a Behavioral Health Provider.
  • Assists Behavioral Health Director in monitoring behavioral health service delivery related to appropriate referrals for behavioral health services: scheduling initial and follow-up appointments as needed; assisting behavioral health and medical providers with referring patients to other services as needed.
  • Assists Behavioral Health leadership in sustaining workflow pertaining to providers administering of behavioral health screenings such as PHQ2, PHQ9, etc.
  • Serves as Behavioral Health community liaison with outside agencies; building community partnerships to share available services
Required Qualifications
  • Bachelor of Health and Human Services-related Degree from an accredited school of social workOR Current Nevada State Social Work Licensure (LSW)
Preferred Qualifications
  • Bilingual Spanish and English.
  • Minimum one year work experience in an outpatient (or Inpatient) behavioral health setting.
  • Skill in operating computer databases
  • Ability to read, write, and calculate mathematical figures.
  • Knowledge of ADA, CPT, and ICD-9-CM coding
  • Ability to communicate in a courteous and professional manner via telephone and in person
  • Ability to understand and respond appropriately, effectively, and sensitively to underserved populations
