Job summary

Nevada Health Centers in Elko, NV is seeking a Licensed Social Worker/Social Services Coordinator to deliver and coordinate psychosocial programs for targeted populations in need of supportive services. Minimum qualifications include a Bachelor’s Degree in Social Work and Nevada licensure or eligibility, with bilingual Spanish and English preferred.

The role collaborates with the care team, documents assessments, and ensures care plan implementation, while offering comprehensive benefits.

Qualifications

• Bachelor’s Degree in Social Work from an accredited school.
• Current Nevada State Social Work Licensure (LSW) or eligible for Provisional Social Worker License and must obtain full licensure within 90 days.

Responsibilities

• Assess and coordinate psychosocial programs for targeted populations.
• Document assessments, plans, interactions, and interventions per NVHC guidelines.
• Maintain knowledge of community resources for patient case management needs.
• Serve as Behavioral Health community liaison and collaborate with care team.
• Assist Practice Manager with insurance preauthorization, scheduling, and referrals.
• Support SBIRT workflow and inform patients about integrated behavioral health services.
• Ensure compliance with 330(h) homeless funding requirements.
• Miscellaneous duties as assigned.
