Catholic Charities Brooklyn and Queens, Inc. seeks a Social Service Coordinator to join POP Management, providing on-site services in senior housing to help tenants age in place.
You will collaborate with building staff to develop programs, coordinate care for high-need cases, and connect tenants with community resources and referrals.

POP Management is an in-house property management company of Catholic Charities Brooklyn & Queens. POP Management is the largest provider of affordable housing in Brooklyn & Queens. POP Management provides affordable housing to low-income seniors and to those with documented history of homelessness, substance abuse, and chronic mental illness. POP Management oversees and manages 26 senior buildings and 4 supportive residences consisting of formerly homeless adults and veterans. The objective of POP Management is to provide high quality housing that responds to the needs of low-income seniors and supportive populations
The Social Service Coordinator works as an integral member of on-site support team providing safe affordable housing for tenants who live in Senior Housing properties managed by POP Management, the housing office of Catholic Charities, Brooklyn & Queens. The Social Service Coordinator (SSC) is responsible for providing a range of on-site services that meet health, psychological or social needs of tenants living independently within the building.
The SSC will be responsible for developing and implementing appropriate and effective communal and individual opportunities that support tenants as they age in place. Targeted support services for those tenants in crisis or need will be developed to support tenants to maintain permanent affordable housing or, where appropriate, are transitioned to more appropriate housing. The SSC will play an integral role ensuring that social services are well woven into the building. The SSC will play a critical role in facilitating volunteer projects in the building and will create opportunities where possible for connections with the neighborhood community to take place.
