Responsibilities

  • Conduct psychosocial assessments and ongoing evaluations of residents’ emotional, social, and environmental needs.
  • Coordinate and participate in care plan meetings, family meetings, and interdisciplinary team discussions.
  • Facilitate discharge planning, including coordination with community resources, hospitals, and outside agencies.
  • Assist residents and families with adjustment to illness, disability, or long-term care placement.
  • Advocate for resident rights and support grievance resolution.
  • Provide counseling and crisis intervention as needed.
  • Maintain accurate, timely, and compliant documentation in the medical record.
  • Ensure compliance with CMS and Minnesota Department of Health requirements.
  • Collaborate closely with nursing, therapy, admissions, and business office teams.
  • Support advance directives, guardianship processes, and resident/family education.

Social Services Coordinator

Position Summary

The Social Services Coordinator is responsible for coordinating and providing psychosocial services to residents and their families in accordance with federal and Minnesota state regulations. This role supports resident well-being, discharge planning, care conferences, and advocacy while ensuring compliance with skilled nursing facility (SNF) requirements.

Essential Duties and Responsibilities
  • Conduct psychosocial assessments and ongoing evaluations of residents’ emotional, social, and environmental needs

  • Coordinate and participate in care plan meetings, family meetings, and interdisciplinary team discussions

  • Facilitate discharge planning, including coordination with community resources, hospitals, and outside agencies

  • Assist residents and families with adjustment to illness, disability, or long-term care placement
