Job summary

The Pavilion at Ocean Point (2) is seeking a qualified Social Work professional to support residents with psychosocial needs and care planning. You will orient new residents to the center, assist with discharge planning, and maintain social records and referrals.

You will coordinate with families and staff, participate in meetings and training, and help oversee care plans and QMPI initiatives to ensure quality resident services.

Qualifications

• Bachelor's Degree in Social Work or a human services field; LSW preferred in TX Centers; one year of supervised health care experience.
• High School Diploma or equivalent.
• Knowledge of Social Resources in the Community.
• Degree in Psycho/Social Health area preferred.

Responsibilities

• Ensure residents' psychosocial and concrete needs are identified and met per requirements.
• Orient residents to the center and assist in discharge planning.
• Implement and update the Resident Care Plan and Social History.
• Maintain records of outside referrals and coordinate residents' financial affairs.
• Communicate needs and plan of care to residents, families and staff.
• Organize and participate in meetings, trainings, and QMPI activities.
• Facilitate Family Council and assist with resident room moves.
• Meet with consultants as required and participate in QMPI.

Principal Responsibilities:

CLINICAL/ADMINISTRATIVE

• Ensure the residents’ psychosocial and concrete needs are identified and met in accordance with federal, state and company requirements.
• Orient residents to the center.
• Discharge planning.
• Implement and update Resident Care Plan and Social History.
• Maintain records of outside referral.
• Assist in coordinating residents’ financial affairs.
• Communicate needs and plan of care to resident, families, responsible parties and appropriate staff.
• Participate in meetings, in-services and training as required.
• Meet with consultants as required
• Organize and Facilitate Family Council (Participate as requested by Council Members).
• Implement and maintain an ongoing Quality Measurement And Performance Improvement Program (QMPI)
• Assist with coordination of resident room moves.
