Summer Temporary Position - Multiple Positions

Discharge Planning: In collaboration with the team, contributes to defining the need for further resources or services.

Communicates data clearly and concisely to the treatment team.

Participates in team meetings and huddles as needed.

Demonstrates understanding of ethnic, cultural, socioeconomic, gender, and lifestyle issues.

Has knowledge of discharge planning needs for adolescents, adults, and the geriatric population.

Coordinates with the treatment team to facilitate optimal discharge plans, considering resources, eligibility, costs, and service types.

Assists in identifying appropriate resources that meet patient and family needs within financial, transportation, or other constraints.

Maintains communication between the treatment team and community agencies, unless delegated.

Keeps current knowledge of community resources and provides accurate information to staff, patients, and families, including limitations.

Involves patients and families in discharge planning, documented in writing.

Completes all documentation promptly, including assessments, releases, and forms within prescribed time frames.

Documents significant encounters and contacts with professionals and community agencies.

Records discharge plans in the EMR.

Performs utilization review functions and acts as the primary liaison with EROs.

Conducts all review procedures: pre-admission, admission, continued stay, and discharge, using hospital and external criteria.

Handles expedited reviews and appeals per department policies and external requirements.

Manages benefit denial processes and ensures notifications are sent.

Maintains accurate records of all cases and reviews.

Provides feedback to the clinical team on benefits and utilization issues.

Maintains necessary computer skills for the role.

Requirements:

Qualifications: BSW

Education: Bachelor’s degree in Social Work or a related behavioral health field.

Training and Experience: Clinical experience in behavioral health and case management required.

Mental Demands: Ability to handle stress in a fast-paced environment and work with multidisciplinary staff and challenging patient populations.
