Summer Temporary Position - Multiple Positions
Schedule: 24/48 - 3 days per week
Job Responsibilities
- Discharge Planning: Collaborate with the team to define additional resource or service needs.
- Communication: Clearly convey data to the treatment team.
- Meetings: Participate in team meetings and huddles as needed.
- Cultural Competence: Understand the impact of ethnic, cultural, socioeconomic, gender, and lifestyle issues.
- Discharge Planning Knowledge: Demonstrate understanding of services and discharge needs for adolescents, adults, and the geriatric population.
- Coordination: Facilitate optimal discharge plans considering resources, eligibility, costs, and service types.
- Resource Identification: Assist in identifying appropriate resources within patient constraints.
- Community Liaison: Maintain communication with community agencies and professionals.
- Resource Knowledge: Keep updated on community resources and communicate availability and restrictions.
- Family Involvement: Engage patients and families in the discharge planning process and document accordingly.
- Documentation: Complete all necessary documentation promptly and accurately, including in the EMR.
- Utilization Review: Collaborate with the treatment team on review functions and serve as liaison with EROs.
- Review Procedures: Perform pre-admission, admission, continued stay, and discharge reviews using hospital and ERO criteria.
- Appeals and Benefits: Manage expedited reviews, appeals, and benefit denials per policies and regulations.
- Record Keeping: Maintain accurate records of reviews and decisions.
- Feedback: Provide regular feedback to the clinical team on benefits and utilization issues.
- Skills: Maintain necessary computer skills.
Requirements
- Qualifications: Bachelor’s degree in Social Work or related behavioral health field.
- Experience: Clinical experience in behavioral health and case management required.
- Mental Demands: Ability to handle stress in a fast-paced environment and work with multidisciplinary teams and diverse patient populations.
