The Care Specialist supports the Social Recreation service line within Disability Services, helping individuals successfully access and maintain person-centered services. This role manages a caseload from referral through ongoing service delivery, conducts assessments and develops Individual Service Plans (ISPs), supports Direct Support Professionals (DSPs), and coordinates with families, Regional Centers, and internal teams. The Care Specialist helps ensure services are delivered effectively, goals are monitored, and program requirements remain compliant.
Business hours are 8:00 a.m. to 6:00 p.m. Pacific Time. Depending on the position and business needs, employees may be required to work any scheduled shift within these operating hours.
